CPT 38222 – Updated BNM

As of Jan. 1, 2018, CPT code 38222 is used when a pathologist performs a bone marrow biopsy
(38221) and a bone marrow aspiration (surgical procedure) through the same incision. G0364
has been deleted for dates of service on and after January 1, 2018.
CPT 2018 added this code to allow for the reporting of diagnostic bone marrow biopsies that are
performed in conjunction with bone marrow aspiration procedure on the same incision.
38220 Diagnostic bone marrow; aspiration(s) only
(Do not report 38220 in conjunction with 38221)
(For diagnostic bone marrow biopsy(ies) and aspiration(s) performed at the same session, use
38222)
38221 Diagnostic bone marrow; biopsy
(Do not report 38221 in conjunction with 38220)
(For diagnostic bone marrow biopsy(ies) and aspiration(s) performed at the same session, use

38222 Diagnostic bone marrow; biopsy(ies) and aspiration(s)
(Do not report 38222 in conjunction with 38220 and 38221)
(For bilateral procedure, report 38220, 38221, 38222 with modifier 50)
(For bone marrow biopsy interpretation, use 88305)

Report this code when a pathologist personally extracts a bone marrow biopsy and aspirations
from a patient. It needs to be noted that you can’t report this or any other code if the pathologist
only assists another physician with the procedure. The pathologist MUST personally extract the
sample.
